Key Responsibilities:
- Deliver tailored nursing care in alignment with physician orders and individualized plans of care.
- Perform comprehensive evaluations, updating and revising care plans as necessary based on patient needs.
- Use your specialized nursing skills to initiate preventive and rehabilitative procedures.
- Keep detailed clinical and progress notes for each visit, ensuring timely communication and coordination of services among team members.
- Counsel patients and their families, providing valuable education and support regarding health needs.
- Participate in in-service programs and share your expertise with team members to foster a culture of continuous improvement.
- Stay informed about Agency policies and procedures to uphold the highest standards of care.
Qualifications:
- Graduate of an accredited school of professional nursing with a current Utah nursing license.
- Minimum of two (2) years of nursing experience preferred, especially in home health or hospice settings.

- Ability to exercise initiative, independent judgment, and proficient critical thinking skills.
